    Employee Appraisal Form

    I have modified an existing Employee Appraisal Form for our use.

    Unfortunately I cannot get it to upload to the site in any format, so I have copied and pasted.

    Any input that you think could be used to improve it or any input on something you think may be missing would be great.

    For any of you that would like a copy, please e-mail me and I would be happy to share. Let me know what word format you would like it in.

    Beside each performance area there is actually 5 boxes representing 1-5 on the scale. It is just not showing up on the cut and paste.

    How would I use a resume to evaluate an employees performance and then create a pay scale to increase them off of?

    I know it looks like junk on here, but that is only because I could not upload it. It actually looks very professional in it's proper format.

    I posted it on another site as I was able to add the attachment there.
