Here is a picture of an exhaust fan in Italy. It is actually in the wall and the vertical duct runs up to the roof inside the wall as well. Not sure how they would clean the vertical, it's almost like they would have to pull the fan out. The kitchen is right on the other side of the fan and the little lateral leading to the kitchen is not connected to the fan, when you close the access door to the fan, it creates a vacuum and sucks the air from the kitchen. Pretty ingenious but an exhaust cleaners nightmare.
