Certificate of Performance

NFPA 96 state that the dates of cleaning are to be on the premise. Some put the dates on the hood as you are showing. I give them a exit report and certificate of cleaning that they can file. The sticker has our name, address and states that the hood cleaning records are on the premise.

We have elected to use this method for two basic reasons. First the date stickers do not hold up well and the return dates are fix and in time you will find a firm date is hard to live up to, the more stores you get. I will send you a set of what I use.

Thanks David, I also do Inspection,After Service follow up report, and a kitchen maitainance report for the kitchen help. All copys left with customer and I file hard copys for any AHJ'S. Mikey
