What Happened Here?

I have seen stuff like that where the dew or rain off a roof hits that area and slowly erodes the concrete but not that bad like in your pictures. It could be that the erosion was bad and becoming a trip hazard so they tore out that section and did a new pour with a different grade of concrete.
 
It looks like it was one large step and then they added a smaller one in between because some old lady broke her hip and her 40 year old son who is a lawyer sued... haha that is my explanation.

The concrete was already drying when poured so It dry to quick leaving air pockets, once the cream of the concrete came off top.

It expose all the holes. Now its a hazard and needs to be taken out or taken down to min of 3inches. Pressure washing will only make it worse, and exposing more holes of air pockets. I have seen a whole sidewalks like this, they should have tested it prior to pouring

I don't think it was on purpose. The concrete is deteriorating. Not sure if it's erosion, per say, but it seems to be a problem and I wonder how it can hold up to power washing.
 
@scottwash...that's an interesting explanation. Makes sense. Was concerned about power washing it. I imagine they will be replacing sometime in the near future.
